Describe your request and identify the email address used for your original inquiry or registration. Include the approximate date and the service you used. Avoid sending identity documents or other sensitive information in an initial message.

If another service collected the information.
For a hotel reservation, payment or registration handled by another provider, contact that provider directly as well. DEC may not hold the records managed by an external service.
